Bone scans are the most sensitive routine imaging modality to try and identify both sclerotic and lytic lesions 1. In most cases they demonstrate increased uptake (hot spot) although occasionally (in very aggressive purely lytic lesions) a photopenic defect (cold spot) may be visible. Bone scintigraphy is widely used to detect bone metastases, particularly osteoblastic ones, and F-18 fluorodeoxyglucose (FDG) positron emission tomography (PET) scan is useful in detecting lytic bone metastases. In routine studies, images are assessed visually. Osteoblastic and osteolytic metastases exhibit increased uptake on bone scans, but very aggressive lytic lesions may not exhibit increased uptake. Bone scan is the preferred and most cost-effective imaging study to evaluate for osseous metastases. Previous treatment is highly relevant and it has been found that although the majority of untreated bone metastases are positive on PET scans and have a lytic pattern on computed tomography (CT), after treatment, incongruent CT-positive/PET-negative lesions are significantly more prevalent and generally are blastic, which presumably reflects a direct effect of treatment.
